KSHSAA Physical Form and Summer Well Checks
The State of Kansas High School Activities Association requires the completion of a specific athletic form to participate on a sports team. This form is available at http://www.kshsaa.org/FORMS/form-ppe.pdf
Please note the form is required annually for each participant. The parent/guardian must complete the front side of the form PRIOR to the child's physical and the completion of the physical examination findings by the doctors in our office. We appreciate your cooperation with these Kansas rules, and welcome any questions you may have about your child.
We certainly encourage you to schedule those summer well checks as soon as possible.
