Chart Copy Fees and Policies
A parent or legal guardian may request, on a one time basis at no charge, the following information: a copy of each child's growth chart, immunization record and last well check-up. Any additional copies of the above information or any other copying requests for that patient will be billed at the rate governed by Kansas law (KSA 65-.4971b). These rates change yearly by law and can be quoted to you over the phone. If you desire a complete copy of your child's chart, payment will be required prior to the release of the copied chart and will be made available for pick-up at the front desk. The copy will be ready within seven days after payment is received and you will be notified by phone. We will need a signed release at that time.
