  • Immunization Records
We remind you to keep track of your child's immunization records.  We are happy to give you a current copy anytime.  There are many schools and programs that require vaccine documentation, so keep a copy in a safe place.  New patients are encouraged to bring a copy of their shot records to their first visit.  We will incorporate that into our medical record here.  Let us know if you have any questions about your child's vaccine history.
