Immunization Schedule

• Immunization Update: The Pneumoccal vaccine has been updated from 7 serotypes to 13. The new Prevnar 13 includes serotype 19A which is the cause of the most severe pneumoccal disease. If your child is older than 15 months of age, he or she received the original Prevnar vaccine and would benefit from a booster with the new Prevnar 13. Ask your doctor about the Prevnar 13 if your child is between 15 months and 5 years.

  Newborn: Hepatitis B (in hospital at birth or during first visit)
  
  
• Two Month: Pediarix (DTaP, IPV, Hepatitis B), Hib, Pneumococcal, Rotavirus
  
  
• Four Month: Pediarix (DTaP, IPV, Hepatitis B), Hib, Pneumococcal, Rotavirus
  
  
• Six Month: Pediarix (DTaP, IPV, Hepatitis B), Pneumococcal
  
  
• Nine Month: Hemoglobin Check
  
  
• Twelve Month: MMR, Varicella, Hepatitis A
  
  
• Fifteen Month: Hib, Pneumococcal
  
  
• Eighteen Month: DTaP, Hepatitis A
  
  
• Two Year: Hemoglobin Check
  
  
• Five Year: Kinrex (DtaP, IPV), MMR (Measles, Mumps, Rubella), Varicella
  
  
• Twelve - Fifteen Year: Tdap, Varicella, Meningococcal, HPV
  
  

Immunization Records
We remind you to keep track of your child's immunization records. We are happy to give you a current copy anytime. There are many schools and programs that require vaccine documentation, so keep a copy in a safe place. New patients are encouraged to bring a copy of their shot records to their first visit. We will incorporate that into our medical record here. Let us know if you have any questions about your child’s vaccine history.
